Port Activation Request
Submit this form when you have an IP address already and just need the port of a new location activated.
Input the PORT INFORMATION as follows for each building:
Location (Required) - Room Number / Building [53-155 / E IV]
and
Port Number (in this format per building) - PORT PLATE Number / PORT NUMBER
for E IV (57-108 / B24)
for BH (E or W-P24)
for E1 (3-156)
for EV - NOT REQUIRED
